Change to the NYS Health Appraisal Form – Effective 7/1/18
We are starting to receive phone calls & portal messages regarding the upcoming change in health examination and screening requirement effective July 1st 2018. The new NYS School Health Examination Form is not required for physicals done prior to 7/1/18. This means that if your child had a physical examination done prior to this date,

PLAN with Families at Genesis Pediatrics
PLAN with Families is a 2-year family-based program that encourages healthy eating habits and promotes healthy lifestyle changes. The goal of the program is for families to learn healthy behaviors that can be maintained long-term. Criteria for participation: One child between the age of 6 and 12 years old and one parent must participate.
