2018-2019 School Requirements for Entrance/Attendance

Below are the requirements for school physicals and immunizations according to grade level per the New York State Department of Health. Please call the office to schedule an appointment for your child(ren).

PLAN with Families at Genesis Pediatrics

  PLAN with Families is a 2-year family-based program that encourages healthy eating habits and promotes healthy lifestyle changes. The goal of the program is for families to learn healthy behaviors that can be maintained long-term. Criteria for participation: One child between the age of 6 and 12 years old and one parent must participate.
