Our office has recently been participating in Quality Improvement projects, including a Patient Medication Reconciliation project and a Lead Testing Initiative. Patient Medication Reconciliation
Goal of the Project: To ensure that we have the most accurate information regarding patient medications based on the The National Committee for Quality Assurance benchmark for Patient Centered Medical Home Certification.

Goal of the Project: To improve our rate of lead testing based on the American Academy of Pediatrics’ recommendation for targeted screening/testing of children 12-24 months old with identified risk.
