We DO NOT accept MVP exchange products. If you have a new MVP product, please confirm that it is not an exchange plan.
Expectations on appointment day:
– Co-pay, HMO and Hybrid plans require that the full co-pay is made at time of service.
– High Deductible plans will now require a $35.00 minimum payment at time of service.
– Government plans do not require a payment at time of service.
Please be prepared to make the appropriate payment on the day of the appointment or you may be charged a $10.00 lack of payment fee. We have a number of alternatives for you to make on-time payments so that you may avoid this additional charge. As always, thank you for your trust in us.
