We are starting to receive phone calls & portal messages regarding the upcoming change in health examination and screening requirement effective July 1st 2018.
The new NYS School Health Examination Form is not required for physicals done prior to 7/1/18.
This means that if your child had a physical examination done prior to this date, the old NYS Health Appraisal form is acceptable.
To assist with this transition, during the 2018-2019 school year schools may accept any health examination form provided.
